Chronic Disease Management(CDM), Prevention Programme(PP) & Opportunistic Case Findings(OCF).

The Structured CDM, PP & OCF consists of three components. 

OCF programme aims to identify those at high risk for preventive programme & those with undiagnosed listed chronic disease for the CDM treatment programme. 

Prevention programme focuses on patients at high risk of cardiovascular disease and diabetes. 

CDM treatment programme comprises of 2 reviews per annum, one with a nurse and one with a doctor. This programme is for patients with
-Asthma
-Type 2 Diabetes
-Chronic obstructive pulmonary disease(COPD)
- Ischaemic Heart Disease
-Cerebrovascular Disease (Stroke/TIA)
-Atrial Fibrillation

Diabetes & Pre Diabetic Clinics 

Diabetic/Pre Diabetic patients being managed in the practice are invited to attend our multidisciplinary diabetic clinic. This serves to ensure optimum care of patients and aims to avoid complications of diabetes.
